All Multi Disk games fail to launch.

Ah i see, So when making folders for multi disc games should it be disc or disk? Does it matter?

I don't feel the DB is jacked.
This is a new install. I just RGH'ed my console the week I posted this thread.
I have also tried a second fresh install, into a new folder, same thing happens.
I ant a dev though so..  

I don't have autoswap enabled in dash lauch, nor did I upload swap.xex anywhere.

I am trying to confirm now, but you are running a VERY old plugin. I believe you should look into making sure your Plugin is the most recent version.
